Erections are a vascular event — they depend on healthy blood flow through small arteries. Because those arteries are narrower than the ones around your heart, they often show trouble first. How the medications work
Sildenafil and tadalafil are PDE5 inhibitors: they relax blood-vessel walls and improve flow when you're aroused — they don't create arousal itself. Sildenafil works on demand over a few hours; tadalafil lasts longer and has a daily low-dose option that removes timing from the equation entirely.
The interaction that isn't negotiable
Combined with nitrates — nitroglycerin and related chest-pain medications — PDE5 inhibitors can drop blood pressure to a life-threatening degree. This is a hard contraindication in FDA labeling, and it's why our intake treats 'I'm not sure if I take nitrates' as a stop sign rather than a maybe.
Recent heart attack or stroke, unstable angina, and uncontrolled blood pressure also route you to in-person care first. A provider who skips these questions isn't being convenient — they're being careless.
