Arousal releases nitric oxide
Sexual stimulation triggers a nitric oxide signal in the blood vessels of the penis. Without that signal, nothing downstream happens.
A messenger relaxes the vessels
Nitric oxide raises cGMP, which relaxes smooth muscle so vessels widen and blood flows in.
An enzyme clears the messenger
PDE5 breaks cGMP back down. When it clears too fast, the response doesn't hold.
The medication slows that clearance
These medications inhibit PDE5, so cGMP persists. Arousal is still required. And because nitrates act on the very same pathway, that combination is never prescribed — it can drop blood pressure dangerously.
