How ED medication works

Sildenafil and tadalafil don't create arousal. They act on what happens after it — which is also why the heart questions are not optional.

  1. Arousal releases nitric oxide

    Sexual stimulation triggers a nitric oxide signal in the blood vessels of the penis. Without that signal, nothing downstream happens.

  2. A messenger relaxes the vessels

    Nitric oxide raises cGMP, which relaxes smooth muscle so vessels widen and blood flows in.

  3. An enzyme clears the messenger

    PDE5 breaks cGMP back down. When it clears too fast, the response doesn't hold.

  4. The medication slows that clearance

    These medications inhibit PDE5, so cGMP persists. Arousal is still required. Nitrates act on the same pathway, so the combination is never prescribed: it can drop blood pressure dangerously.

Because ED medication and certain heart conditions or medications — nitrates especially — are a dangerous combination. If it isn't safe, nothing is prescribed and you are told why.

Prices are if prescribed. If nothing is prescribed, the medication portion is refunded; the $32 consultation is not.
