Telehealth informed consent

This explains what you are agreeing to when you start an online visit with ReviveRx Plus. It is written to be read, not to be scrolled past.
How your care is delivered
Your visit happens in one of two ways, depending on your state and your treatment. Either it is a short live video conversation with a licensed provider, or it is asynchronous — sometimes called store-and-forward — where you answer questions about your health in writing and a licensed provider reviews your answers and decides. In both cases no physical examination takes place.
A provider may decide your situation cannot safely be assessed this way. If so, they will tell you and direct you to in-person care.
What this cannot do
- It is not a substitute for a physical examination, and some conditions cannot be diagnosed without one.
- A provider is working from what you tell them. Incomplete or inaccurate answers can lead to a wrong decision, which is why the questions ask what they ask.
- It is not an emergency service and is not monitored in real time. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
- Technology can fail. A delay or an outage could postpone review of your visit.
You may not receive a prescription
Completing a visit does not entitle you to medication. The provider exercises independent clinical judgment and may decide that no treatment is appropriate, that a different treatment is, or that you should be seen in person. A decision not to prescribe is a clinical outcome, not a failure of the service.
ReviveRx Plus does not practice medicine and does not influence that decision.
Your alternatives
You are free to seek care in person instead, at any time, and to stop a visit at any point. You may withdraw this consent by writing to support@reviverxcare.com. Withdrawing consent does not affect care already provided.
Your information
Your answers are shared with the licensed provider reviewing your visit and with the pharmacy that would fill a prescription, if one is written. How health information is handled is set out in the Privacy policy and the Notice of Privacy Practices.
Email and text messages are not encrypted. Other people with access to your inbox or phone may be able to read what we send you.
Licensure
Care is available only in states where a provider working with us holds a licence. Your state is the first question of every visit, and if we are not licensed where you are we will tell you before you go any further.
Complaints
If you have a concern about your care, write to support@reviverxcare.com. You also have the right to complain to the medical board of the state in which your provider is licensed; several states require that route to be published here, and the specific board contacts will be listed on this page before launch.
