This notice describes how medical information about you may be used and disclosed, and how you can get access to it.

A Notice of Privacy Practices is issued by the health care provider entity, not by the company that runs this website. Your care will be provided by an independent professional entity, which is the HIPAA covered entity. That entity is being formed, and its notice will be published here — signed, dated and in full — before any patient is seen. ReviveRx Plus acts as its business associate for platform services.

What the notice will cover

How protected health information is used for treatment, payment and health care operations
When information may be disclosed without your authorization, and when your authorization is required
That your intake answers are shared with the provider reviewing your visit, and with the pharmacy filling a prescription if one is written
The entity's duties, and how it will notify you of a breach

The rights it will describe

To inspect and obtain a copy of your medical record
To request an amendment to it
To request restrictions on how it is used or disclosed
To request that we communicate with you by a particular method or at a particular address
To an accounting of certain disclosures
To a paper copy of the notice, on request
To complain — to the entity's Privacy Officer, or to the US Department of Health and Human Services — without retaliation

The Privacy policy explains how this platform handles your information today, and the Consumer health data notice covers rights under state health-data laws. Questions can go to privacy@reviverxcare.com. A named Privacy Officer for the professional entity will be published alongside the full notice.
