Privacy policy

Health information is different, and we treat it that way. These are the commitments this policy will formalize:
- Your intake answers are collected for one purpose: review by the licensed provider handling your care.
- We do not sell your personal or health information.
- We do not place health details or condition names in URLs, and we keep advertising pixels off intake and health pages.
- Vendors that touch health information must sign the agreements required to protect it before they touch it.
- You can ask what we hold about you, and ask us to delete it, subject to medical record-keeping law.
Who is the covered entity
HIPAA applies to health care providers, not to every company that handles health information. Your care will be provided by an independent professional entity and the licensed clinicians it contracts — that entity is the HIPAA covered entity for your medical record, and it issues its own Notice of Privacy Practices.
ReviveRx Plus runs the platform on that entity's behalf and acts as its business associate. Any vendor of ours that touches protected health information signs a business associate agreement before it does so — and where a vendor will not sign one, we do not use it for anything involving your health information.
