The same pathway as sildenafil — inhibiting PDE5 so cGMP persists and vessels stay relaxed — but tadalafil clears from the body more slowly. That's what makes a low daily dose possible, and why the as-needed version has a longer window. Arousal is still required either way. The approved tablet is also labeled for the urinary symptoms of an enlarged prostate, which is why a provider may ask about those.

Reasons it may not be prescribed
- Anyone taking nitrates — the same absolute contraindication as sildenafil, and it lasts longer because the drug does
- Recent cardiac events, unstable angina or uncontrolled blood pressure
- Anyone on alpha-blockers or specific antivirals without a full medication review
These are the same checks your provider runs in the visit. If one applies, they will tell you plainly — and the medication portion of what you paid comes back automatically.
Important safety information
Headache, indigestion, back or muscle ache, flushing and nasal congestion are the common ones — back ache is more associated with tadalafil than with sildenafil. Seek emergency care for an erection lasting more than four hours, or sudden vision or hearing loss.
