Will I definitely be prescribed this?
No. A consultation does not guarantee a prescription. A licensed provider reviews your allergy history and prescribes a replacement only where there is a genuine risk of anaphylaxis — and if you have never been assessed for that in person, they will route you to an allergist for the plan this device sits inside. If prescribed, the prescription is sent to a local pharmacy and you pay for the device there; what you pay here is the consultation.
What do I do after using it?
Call 911, lie down with your legs raised unless you are struggling to breathe, and if there is no improvement in five to fifteen minutes use the second device. Go to the emergency department even if you feel fine — a second wave can follow hours later, and the used device should be replaced the same day.
Brand or generic — does it matter?
The drug and dose are the same; the FDA-approved generics contain the same epinephrine. The devices differ in how they are triggered and held, so learn the one you are given rather than assuming it works like your last one.
How it works
Anaphylaxis is the immune system's mast cells emptying at once: histamine and other mediators flood the body, blood vessels dilate and leak so pressure falls, the airways tighten, and the lips, tongue and throat can swell. Epinephrine is the one drug that reverses all of that at the same time, because it acts on three kinds of receptor. On alpha-1 receptors it constricts blood vessels, which lifts blood pressure and shrinks swelling; on beta-1 receptors it strengthens and speeds the heart; on beta-2 receptors it relaxes the airway muscle and, importantly, signals the mast cells to stop releasing more mediator. Given by auto-injector into the outer thigh muscle it is absorbed within minutes. It buys time — it does not end the emergency. A reaction can return after the drug wears off, a second dose may be needed if there is no improvement in five to fifteen minutes, and a delayed second wave can follow hours later, which is why every use is followed by a 911 call and observation. This visit is for keeping a working device in reach: renewing the prescription for someone who already has a diagnosis and an action plan, and confirming they carry two.

It won’t be prescribed if
- Anyone having an allergic reaction right now — swelling of the lips, tongue or throat, trouble breathing, hives with dizziness or faintness — use your auto-injector if you have one and call 911; this visit cannot help in an emergency
- A first severe allergic reaction that has never been evaluated in person — you need an allergist's assessment, testing and a written anaphylaxis action plan; the provider may prescribe a device to carry in the meantime, but this visit does not replace that in-person plan
- Under 18 — a child's device is dosed by weight and prescribed with their paediatrician or allergist
- Heart disease, an irregular heartbeat, uncontrolled high blood pressure, or medications such as MAO inhibitors or certain antidepressants — not a reason to be without a device, since epinephrine is still the treatment for anaphylaxis, but the provider needs to know and to counsel you
- Someone seeking epinephrine for any purpose other than a diagnosed allergy risk — it is not prescribed for asthma, panic or weight loss

After your visit
If prescribed, the prescription is sent to a local pharmacy you choose and you pay for the device at the counter — brand or an FDA-approved generic, depending on what the pharmacy stocks and your insurance. What you paid here covers the consultation.
When you collect it
Read the instructions for the specific device you receive, since the brand and generics are triggered differently; check the expiry date and the window on the device — the solution should be clear. Practise with the trainer if one is included, and tell the people around you where it is.
Ongoing
Carry two. Replace at expiry or after any use. Keep it at room temperature — not in a hot car or a freezer. Every use is followed by a 911 call, even if you feel better.
Safety
Side effects, before you decide
After a dose, the expected effects of adrenaline: a racing or pounding heart, shaking, anxiety, headache, pale skin, sweating, dizziness and nausea, all of which pass. Serious effects are rare but real — heart rhythm disturbance, a spike in blood pressure and, in people with heart disease, angina. An accidental injection into a hand or foot can cut off blood flow to the area and needs emergency treatment. Rarely, serious skin and soft-tissue infections have been reported at injection sites. None of these is a reason to withhold epinephrine in anaphylaxis.
