Will I definitely be prescribed this?
No. A consultation does not guarantee a prescription. A licensed provider reviews your symptoms, your history and the red flags above and prescribes only where the picture clearly fits a yeast infection or bacterial vaginosis you have had before; where it might be a sexually transmitted infection, a first episode, or pregnancy, they will route you to testing or in-person care and say why. If prescribed, the prescription is sent to a local pharmacy and you pay for the medication there; what you pay here is the consultation, whether or not a prescription follows.
Can't I just buy the over-the-counter yeast treatment?
You can, and its label is written for exactly one situation: a yeast infection you have had diagnosed before and recognise. The point of this visit is the other half — bacterial vaginosis, which an antifungal does nothing for, and the infections that mimic both. Studies of self-diagnosis find that many people treating themselves for yeast have something else.
Does my partner need treatment?
Not for bacterial vaginosis or a yeast infection in a partner without symptoms — routine partner treatment has not been shown to help with either. If testing finds trichomoniasis or another sexually transmitted infection, partners are treated, and that is a different visit.
How it works
A healthy vagina is kept mildly acidic by lactobacilli, and the two common infections are two different ways that balance tips. A yeast infection is an overgrowth of Candida, usually after antibiotics, in pregnancy, with diabetes or for no clear reason: thick white discharge, intense itch and soreness. Azole antifungals — fluconazole taken by mouth as a single dose, or clotrimazole and miconazole placed vaginally — block the fungal enzyme that builds ergosterol, the essential component of the yeast cell membrane, so the membrane fails and the population collapses. Bacterial vaginosis is not one organism invading but the lactobacilli losing ground to a mixed community of anaerobic bacteria: the pH rises and the result is a thin grey discharge with a fishy smell, often without much itch. Metronidazole is taken up by those anaerobes and converted inside them into fragments that break their DNA, sparing the oxygen-using cells around them; clindamycin binds the anaerobes' ribosomes and stops protein synthesis. The two conditions need opposite treatments, and trichomoniasis and other sexually transmitted infections can mimic either, which is why the provider asks about partners, previous diagnoses and the exact character of the symptoms — and why an unclear picture is sent for testing rather than guessed at.

- A thin discharge with a fishy odour you recognise from a previous bacterial vaginosis diagnosis
- You want a provider to decide which of the two it is — or whether it needs a swab — rather than guessing at the pharmacy
It won’t be prescribed if
- Pelvic or lower abdominal pain, fever, or pain during sex — pelvic inflammatory disease needs in-person examination the same day; call 911 if the pain is severe or you feel faint
- Pregnancy or possible pregnancy — a hard stop for this visit; vaginal infections in pregnancy are managed with your obstetric provider, and single-dose oral fluconazole is avoided
- A first-ever episode that has never been diagnosed in person, or symptoms that did not respond to a recent course — needs a swab and an examination
- A new partner, a partner with symptoms, or any possible exposure to a sexually transmitted infection — needs testing, because trichomoniasis, chlamydia and gonorrhoea mimic these symptoms and are treated differently
- Sores, blisters or ulcers, or bleeding between periods or after sex — needs examination
- Four or more yeast infections in a year — recurrent infection needs a culture and a longer plan, not another single dose
- A known allergy to fluconazole or other azoles, to metronidazole, or to clindamycin — a hard stop for that medication
- Male anatomy — this exam is for vaginal symptoms
- Under 18 — online treatment here is for adults

Days 1–3
Yeast symptoms usually ease within one to three days of the single fluconazole dose or the start of a vaginal antifungal. Bacterial vaginosis typically improves within a few days of starting metronidazole or clindamycin — finish the course, and no alcohol during metronidazole or for three days after.
Days 4–7
Symptoms that have not settled within a week, or that return soon after, mean a swab and an in-person assessment rather than a second course by video.
Safety
Side effects, before you decide
Fluconazole: headache, nausea and abdominal pain; rarely liver injury or a heart rhythm effect, and it interacts with many medications including warfarin and some statins — the provider needs your list. Vaginal clotrimazole and miconazole: local burning or irritation, and the oil-based products weaken latex condoms and diaphragms. Metronidazole: a metallic taste, nausea, dark urine, and a severe reaction with alcohol; rarely numbness or tingling in the hands and feet that means stop. Clindamycin cream: a yeast infection afterwards is common, it weakens latex condoms for several days, and rarely it can cause Clostridioides difficile colitis even used vaginally — persistent or bloody diarrhoea means stop and contact a provider.
