Will I definitely be prescribed this?
No. A consultation does not guarantee a prescription. A licensed provider asks why you are vomiting before anything else, and prescribes only where the cause is one that is safe to manage at home — if your story suggests dehydration, an abdominal emergency, pregnancy or a heart or brain cause, they will route you to in-person care and say why. If prescribed, the prescription is sent to a local pharmacy and you pay for the medication there; what you pay here is the consultation.
Which of the three will I get?
The provider chooses by your history: ondansetron where sedation would be a problem and your heart rhythm allows it; promethazine where drowsiness is acceptable or motion is part of the picture; prochlorperazine where the dopamine pathway is the one to block. They will not prescribe all three.
What about nausea in pregnancy?
It needs its own plan, with your obstetric provider, and this visit does not cover it. The choice of medication in pregnancy weighs factors that belong in that conversation, and vomiting severe enough to cause dehydration in pregnancy is assessed in person.
How it works
Vomiting is coordinated by a centre in the brainstem that takes input from three places: the gut, where irritation releases serotonin from cells in the intestinal wall onto the vagus nerve; a trigger zone at the base of the brain that sits outside the blood-brain barrier and samples the blood for toxins and drugs; and the balance organs of the inner ear. The three medications here each block a different input. Ondansetron blocks the serotonin 5-HT3 receptors on the vagus nerve and in the trigger zone, which is why it suits gut-driven nausea and does not sedate. Promethazine blocks histamine H1 and acetylcholine receptors, which quiets the inner-ear and brainstem pathways — good for motion-type and general nausea, at the cost of drowsiness. Prochlorperazine blocks dopamine D2 receptors in the trigger zone, the pathway most drugs and toxins use. All of them treat the symptom rather than its cause, which is why the provider's first question is why you are vomiting: a stomach virus that will pass in a day or two is one answer, and appendicitis, a bowel obstruction, pregnancy, a head injury, diabetic ketoacidosis or a heart attack are others that no tablet should be allowed to mask.

It won’t be prescribed if
- Vomiting blood or material that looks like coffee grounds, black or bloody stools, severe abdominal pain, a rigid or swollen belly, or pain settling in the lower right — call 911 or go to an emergency department
- Chest pain, pressure or pain spreading to the arm or jaw with nausea, or a severe sudden headache, a head injury, a stiff neck or confusion — emergency care; nausea can be the presenting symptom of a heart attack or a brain emergency
- Signs of significant dehydration — dizziness on standing, no urine for eight hours, a racing heart, dry mouth, or inability to keep any fluid down for more than a day — needs in-person assessment and possibly intravenous fluids
- Pregnancy or possible pregnancy — nausea of pregnancy needs its own plan with your obstetric provider; this visit is not the route
- Diabetes with high blood sugars and vomiting — diabetic ketoacidosis is an emergency
- A known heart rhythm problem or long QT, or medications that prolong the QT interval — ondansetron may be excluded; Parkinson's disease — the dopamine-blocking options are excluded; the provider needs your full medication list
- A known allergy to ondansetron, promethazine, prochlorperazine or related phenothiazines — a hard stop for that medication
- Vomiting that has lasted more than two days, or that keeps returning — needs a diagnosis rather than another course

Hours 1–24
Ondansetron's dissolving tablet works within about half an hour without needing to be kept down. Sip small amounts of fluid often — an oral rehydration solution is what your body actually needs. Promethazine and prochlorperazine will make you drowsy: no driving.
Days 2–3
A stomach bug typically settles within a couple of days. Vomiting that persists, worsening pain, fever, or any of the red flags above at any point means in-person care.
Safety
Side effects, before you decide
Ondansetron: headache, constipation and tiredness; it can prolong the heart's QT interval, which matters with certain heart conditions and medications, and combined with antidepressants and some other drugs it can contribute to serotonin syndrome. Promethazine: marked drowsiness, dry mouth, blurred vision, dizziness and, in older adults, confusion; it must never be given to a child under two. Prochlorperazine: drowsiness, restlessness, and muscle spasms of the neck, jaw or eyes that need prompt treatment; rarely a dangerous reaction with high fever and rigidity. Both promethazine and prochlorperazine can cause movement disorders that become persistent with prolonged use — the reason a course is short.
